Register For a New Account

Becoming a registered user of DASA-TA is a two step process. Before you use this system, you must complete both steps. After that, the site administrator will activate your account.

Step 1:

  • Download the PDF registration form.
  • Print the form and fill it out with your user information. Check 'Treatment Analyzer' in the User Type box.
  • Obtain the appropriate signature form your supervisor at the bottom of the form.
  • Fax the completed, signed form to the TARGET help desk at (360) 586-0702.

Step 2:

Provide the requested information below. An asterisk denotes a required field.
